The definition
Verification Overrun is the hazardous condition in which a person is held accountable for a verification that exceeds the capacity to verify under the conditions provided.
The condition appears where a person must interpret, review, approve, or intervene, so it can look like a human-performance problem. But the requirement, function allocation, evidence, time, staffing, permissions, and authority are set upstream through work design.
A missed defect does not by itself establish Verification Overrun. A functional gate can miss an error. Overrun is diagnosed when human verification accountability is retained, no functional gate governs the consequential transition under the stated operating condition, and work proceeds.
The work sets the demand
Verification Requirement is what a rigorous check demands at a specified workflow position and operating condition. Verification Capacity is what the workflow supplies there.
The four dimensions are conjunctive and interdependent:
- Knowledge — usable work-system, workflow, and subject-matter competence.
- Evidence — traceability, observability, context, history, exclusions, transformations, and anchors.
- Time — protected time, attention, and staffing within the intervention period.
- Authority — practical power to pause, stop, return, correct, reject, defer, constrain, or escalate.
Surplus in one dimension does not erase a required shortfall in another. Routing more work to a human does not create capacity.
Three transition states
- Covered — a functional gate governs passage under the stated operating condition, and the assigned human, automated, or hybrid verification is properly provisioned.
- Uncovered — no functional gate governs the transition, and human verification accountability is not retained.
- Overrun — no functional gate governs the transition, work proceeds, and human verification accountability is retained.
A safe stop, constraint, deferment, return, transfer, or escalation is a functioning response. It is not Verification Overrun.
Design response
Correct the work design through Provision, Preservation, and Placement: supply the missing capacity, preserve the friction and slack required for correction, and place the gate at a consequential choke point inside the Verification Window.
Naming history
The hazard was originally deposited under the working title Cognitive Overrun. It was renamed because verification capacity belongs to the conditions of work, not the internal state of the worker. The earlier deposit remains part of the concept's lineage.
